Tryout Registration
By signing this form, I give permission for my child to participate in Maryland Belles basketball tryouts. I acknowledge the potential risk of injury. I do hereby agree to waive, release, absolve, indemnify and hold harmless the Maryland Belles organization, it’s members and officers from any and all claims or damages, cost of expenses arising out of any injuries, damages, or other losses, whether personal or property, sustained by my child or any party I am responsible for. I hereby authorize the Maryland Belles organization to seek medical treatment for my child as needed. I give the Maryland Belles permission to use my child’s name and likeness for publicity and to display images on its website and other media.
